The Exclusive Care Plan has included the Centers of Excellence approach to medical care to address your individual needs for certain medical and mental health conditions. Centers of Excellence (COE’s) are specific facilities and providers that have been designated by Exclusive Care to provide outstanding care for specific conditions and procedures. COE’s are characterized by exemplary results in the area of specialty you may need.
If you should need an organ transplant, heart surgery procedures, cancer care, joint replacement surgery, mental
health or chemical dependency services, we will work with you to find the closest possible location to your home that will meet our COE’s criteria and provide you with the care you need.
If you should need care for any conditions that qualify for care at a COE, please call our Medical Management team to assist you and your physician in making arrangements at one of our designated COE’s.
The Exclusive Care Center for Optimal Health is a clinic that focuses on the management of diabetes with a multidisciplinary approach. It was designed and is staffed to provide the highest quality screening and ongoing care for County employees and others who have been diagnosed with diabetes, or those who are at risk of contracting this disease. The Center for Optimal Health is also recognized by the National Committee for Quality Assurance (NCQA) and the American Diabetes Association (ADA) for providing the highest-level diabetes care as measured in their Diabetes Recognition Program (DRP). This voluntary program is designed to recognize physicians and other clinicians, who use evidence-based measures and provide excellent care to their patients with diabetes.
They offer a convenient on-site physician services, same day visits for evaluation and treatment for non emergency services, friendly Endocrinologists & staff, testing for diabetes and Pre-diabetes (early stages of diabetes), an individualized plan of care for patients with diabetes, as well as assistance with diet and nutrition from their on-site dietitian.
Other services include education for you and your family to help manage your diabetes, lower cost medications with use of the Rubidoux pharmacy, assistance with managing medications (Patients receive a medication bag and Diabetes Health Record Booklet), flu vaccinations (available October through March) & Pneumonia vaccinations at no cost to you. They welcome new patients ages 18 and up.
Also available at the Center for Optimal Health are Primary Care services administered by Exclusive Care's new Primary Care Physician, Dr. Amy Daguio. Members may select Dr. Daguio as their Primary Care Physician by calling Exclusive Care's Membership Services Department at (800)962-1133, option 1.

The Rubidoux Pharmacy offers services above and beyond the dispensation of medications such as consultation with the Pharmacy Director, partnerships with physicians for the best possible utilization of prescriptions, and innovative packaging to ensure members take the right dosage at the right times. To help bring more savings to our members, the Rubidoux Pharmacy has a mandatory mail order program for maintenance medications.

Remembering important checkups and lab tests just got easier
The free Email Reminder Service from Quest Diagnostics takes the hassle out of remembering when to schedule the examinations and tests you and your doctor have agreed you need.
With one easy online registration, you can receive ongoing reminders for:
· Diabetes
· Heart Disease
· Women’s Health
· Men’s Health
You can select how often to be alerted and whether your email should be in English or Spanish. Custom reminders are also available for the test or topic that is most relevant to you.
Each email you receive will list upcoming tests and remind you to talk to your doctor. If your doctor says it is time for these tests, he or she can draw your blood for testing, or you can use a link in your reminder to schedule an appointment at a Quest Diagnostics Patient Service Center.
Your reminder will also include links to important topics that can empower you and your family to lead healthier lives. Learn about preventing skin cancer, dealing with food allergies, understanding the warning signs for chronic kidney disease, and more.
