Our goal is to help you find outstanding facilities for your care that will lead to favorable outcomes and provide you with the tools you need to get healthy and well. We have included in your plan the Centers of Excellence approach to medical care to address your individual needs for certain medical and mental health conditions.
Centers of Excellence (COE’s) are specific facilities and providers that have been designated by Exclusive Care to provide outstanding care for specific conditions and procedures. Centers of Excellence are characterized by exemplary results in the area of specialty you may need. These facilities and physicians are recognized for excellence by national accreditation organizations based on documented, evidence-based measures of quality and outcome. Our network of outstanding facilities and physicians covers the entire United States to provide you convenient, quality care.
If you should need an organ transplant, heart surgery procedures, cancer care, joint replacement surgery, mental health or chemical dependency services, we will work with you to find the closest possible location to your home that will meet our Centers of Excellence criteria and provide you with the care you need.
Here at Exclusive Care, your health and the health of your family is our number one priority. If you should need care for any of the conditions that qualify for care at a Center of Excellence, please call our Exclusive Care Medical Management team at (800) 962-1133/option 3 to assist you and your personal physician in making arrangements at one of our designated Centers of Excellence. Your personal physician will send us a referral for the requested service and we will make the appropriate arrangements by coordinating with you and your physician directly.
Conditions that Exclusive Care has designated that are best performed at facilities with demonstrated excellence in the condition or procedure include:

Heart and blood vessel conditions such as bypass surgery, cardiac catheterization to open blocked arteries in the heart, heart valve replacement, and surgery to open blocked blood vessels in the neck that may cause a stroke

Cancer care including lung, colon, breast, pancreas and prostate cancer

Joint replacement of the knee and hip

Mental health care for problems such as memory impairment, Alzheimer’s disease, pain management, drug and alcohol dependency and depression
