Medicare Eligible Public Agency Retirees

Designed with you in mind. The Exclusive Care Select Plan is available for public agency eligible retirees enrolled in Medicare.  Medicare pays first, the plan pays second. The Select Plan also provides services that Medicare doesn’t cover, such as yearly physical exams.

To accommodate your active lifestyle, the plan gives you the flexibility to seek care from any provider who accepts Medicare assignment. Exclusive Care Select Plan is like a Preferred Provider Organization (PPO) plan that offers two tiers of coverage which you can access at any time.

  • Tier 1: The Exclusive Care Network (In-Network) - Offers the highest benefits and lowest out-of-pocket costs. Provides comprehensive coverage through a focused network of physicians, medical groups and hospitals that have contracted with Exclusive Care. Click on our online Exclusive Care Network provider directory to see what is available in your area.  

  • Tier 2: MultiPlan/PHCS Network or any provider that accepts Medicare Assignment - This tier provides comprehensive, affordable benefits for members who live or travel outside the Exclusive Care network.  Our MultiPlan/PHCS Network provides excellent coverage throughout the state, as well as coverage anywhere in the country. You may also access a physician who is not part of the MultiPlan network as long as they accept Medicare Assignment. If there are no Tier 1 providers in your area, Tier 2 will be your preferred provider network.
